Diabetic Foot Care

We provide a variety of services for  diabetic foot care and prevention.  Our diabetic patients have a yearly vascular ( arterial and venous) evaluation done by Dr. Birjandi with Imex machine.  They are also evaluated for diabetic neuropathy by having a 45 minutes exam performed by our on site Neurology technician that performs the test based on our need and request.  Routine diabetic foot care is also done by Dr. Birjandi every 8 weeks, and can be provided more often if needed.  This consists of nail, callus, and corns debridment.  Also diabetic ulcers are debrided as often as needed and are kept under close supervision by the doctor.  For treatment of diabetic neuropathy, Microvasc physical  therpy is provided in our office to the  patients and the frequency of treatment can be monitered.   Diabetic shoes with custum diabetic inserts are also provided yearly or as needed and are fabricated by Dr. Comfort labratory.  The impressions for the inserts, shoe measurment and shoe selection is done in our office.  Catalog and shoe samples are provided so the patient can decide on the shoe style and color.

All above services are covered by most insurances including Medicare/private or Medicare-Medical.  The above services are provided and covered by insurance companies because they are preventive care and hence saves insurance companies a lot of  money from preventing patients being  admitted to the hospitals for an infected ulcer, amputations  due to neuropathy, ill fitting shoes and poor diabetic foot care and education.

The IMEXLAB 9000/9100 is a non-invasive diagnostic system designed to detect peripheral vascular diseases. This test is performed on our diabetic or non-diabetic patients with peripheral vascular disease. This test is covered by medicare and most private insurances.

The IMEX machine performs the following types of automated tests:

  • ABI (Ankle/Brachial Index) Test with Doppler—screens a patient for aterial disease in the legs by using Doppler ultrasound.
  • ABI (Ankle/Brachial Index) Test with PPG—screens a patients for arterial disease in the legs by using photoplethysmography
  • Segmental Test with Doppler— isolates and characterizes the severity of arterial disease in the legs by using Doppler ultrasound
  • Segmental Test with PPG—isolate and characterizes the severity of arterial disease in the legs by using photoplethysmography
  • Digital Test—evaluates the arterial blood flow in the hands or feet
  • Venous Reflux Test—provides an indication of incompetent valves in the superficial and deep veins of the legs
  • Venous Doppler Tests—compliments the Venous Reflux and Venous Ourflow Tests
  • MVO— (Maximum Venous Outflow) Test for DVT—evaluates the sufficiency of the venous outflow in the legs

dr comfort logoOur diabetic patients benefit from a variety of stylish diabetic shoes. A foam impression is taken from one patient’s foot for fabrication of a custom diabetic foot insert that is then placed in the shoe by the laboratory (Dr. Comfort). The goal of this service is prevention and not simply offering shoes.

By insuring that our diabetic patients have proper protective foot gear, we prevent potential foot ulcerations and infections that can lead to hospitalization and potentially, in some cases, leading to amputation. The diabetic shoes and diabetic inserts are covered by medicare and most private insurances.
